Open Disclosure Handbook

The Open Disclosure Handbook provides practical guidance for staff involved in open disclosure following a patient safety incident.

Use the handbook to prepare for, conduct and follow up open disclosure discussions with patients, carers and families.

It covers:

  • what open disclosure is, including why it is important
  • how to initiate and conduct clinician disclosure, and formal open disclosure
  • information on apologising and saying sorry
  • how to support staff involved in patient safety incidents
  • additional information on conducting open disclosure in specific situations and care settings
  • helpful resources including example wording and checklists.
