Clinical Emergency Response System

The Clinical Emergency Response System (CERS) is a core part of the NSW Deteriorating Patient Safety Net System. It sets out formal escalation pathways for staff, patients, carers and families to get timely clinical assistance when a patient shows signs of deterioration.

Raise It (formally known as REACH) helps patients, carers and family members raise concerns about clinical deterioration using a simple, consistent process. Partnering with consumers and having processes for patients, carers and families to directly escalate care aligns with the Australian Charter of Healthcare Rights and the National Safety and Quality Health Service Standards.

Kyran Day's story

Kyran Day's story shows why listening and responding matters when carers and families raise concerns about deterioration. It is part of mandatory training for all NSW Health staff.
