Patient Based care: the LHD level chart been updated. All other text and materials related to this indicator remains unchanged. A revised pdf of this section has been uploaded.

Falls-related Injury Hospitalisations: The text in 'findings', 'implications', 'what we don't know' and 'definitions' sections have been amended in line with the data published by Falls and Injury Prevention Group, Neuroscience Research Australia on Mon 18 Nov. The charts have also been revised.Revised text has been reviewed by this group. All other text and materials related to this indicator remains unchanged. A revised PDF of this section has been uploaded.

SCIDUA (Special Committee Investigating Deaths Under Anaesthesia): The text in 'findings', 'implications' and 'what we don't know' sections have been amended ('Findings', 'Implications' and What we don't know' tabs). All other text and materials related to this indicator remains unchanged. A revisedpdf of this section has been uploaded.

CHASM (Collaborating Hospitals' Audit of Surgical Mortality): The Chart CM01 (on Charts' tab) now includes St Vincent's Health Network data. A revised pdf of this section has been uploaded. The overall text of this indicator remains unchanged.

During the embargo period, some LHDs reported issues with site access and/or specifically access to the dynamic charts in some instances. Please be assured we are continuing to work on this issue.
