Root Cause Analysis Methodology Training

Program overview

The aim of the Root Cause Analysis (RCA) Team Leader Training is to provide the skills an RCA Team Leader requires to lead and engage an RCA team, and to improve the quality of RCA investigations and subsequent RCA reports across the state. The program focuses on developing skills to:

  • Effectively lead a team of clinicians through the process of undertaking an RCA investigation
  • Address the challenges of collecting and managing evidence under privilege and determining and documenting root cause and contributory factors
  • Develop appropriate system improvement recommendations
  • Write a well-constructed RCA report

Learning objectives

  • Describe the steps a RCA Team Leaders needs to undertake to effectively lead a team of clinicians through an RCA investigation
  • Describe the challenges of collecting and managing evidence under privilege and determining and documenting root cause and contributory factors
  • Develop appropriate system improvement recommendations
  • Recognise the components of a well-constructed RCA report

Evaluation of program

Participants will be asked to give feedback at the conclusion of the program survey. It is an expectation that all participants complete the course evaluation survey.

Assessment

It is expected that participants will participate fully in the practical sessions and be fully engaged.

CEC certificate

Certificates will be issued to participants who complete the program.

CPD points

Participation in this program will qualify participants for continuing professional development hours, as required by their respective National Board.
